Barton Advanced Care helps patients develop an optimal care plan catered to their individual health and comfort needs. Sometimes called a palliative care plan, a patient with a serious or terminal disease has a personalized team of specialists to support his or her medical, social, emotional, and spiritual wellbeing. This multi-disciplinary team focuses on care plan development around the patient's personal goals; incorporating the patient's family and support system; to ensure appropriate, informed medical decision making.
